On attempt to save document you get:

The Field Name 'x1bb066ca8993e8ce' specified in the ReturnDocumentItemsFieldArrayIndex() function was not found.

Finally tracked down to 3rd party application by Hogan Data causing the issue: http://datapluscrmsolutions.com/

Uninstalled and issue is resolved.
