In salesforce.com while trying to lookup an account the user was getting Fault Code (1112), session ID missing or invalid. (You may also note that the Saleforce Login gets stuck in a continous "loop.")
The session with salesforce.com was dropping and QuoteWerks was not trying to restore it prior to looking up the contact.
A: Upgrade to Version 3.0 Build 37.11 or higher.
B. If you are currently on version higher than 3.0 Build 37.11, please upgrade to the next higher build. If higher build not available, please delete the QW30.exe (in version 3.0) or the QW4.exe (in version 4.0) along with the NSetup.exe and reinstall the same version and build of QuoteWerks.
Note: One customer that experienced this is a later build of 4.0 reported that he simply changed the user's password in SalesForce.com and then changed it in QuoteWerks and this resolved the issue.
